Medical Forms

Emergency Data Information Sheet - required for all students each year; completed by parent/guardian; sent home in the summer mailing. If yours has been misplaced, you may click here for a blank form.
2010-2011 Health Appraisal Form - required for all students new to the school, students moving to a new learning community, and for middle and Upper School students participating in the athletic program; completed by parent/guardian and physician
Allergy Emergency Action Plan - required for any student with severe allergies; completed by parent/guardian and physician
Asthma Emergency Action Plan - required for any student with asthma; completed by parent/guardian and physician
Authorization for Long-Term/Short-Term Prescription Medication - required for any student taking prescription medication during the school day; to be completed by parent/guardian and returned with labeled medication
Authorization for Over-the-Counter Medications - ECP-Grade 1, Grades 2-4, Grades 5-12 - required

Note: The State of Michigan requires all sixth grade students to have documentation showing they have received the immunizations listed here. If your child is entering sixth grade, please verify the immunization record and have the Health Appraisal Form updated as necessary.
