Medical Forms 2013-2014

• Emergency Data Information Sheet - required for all students each year; completed by parent/guardian; sent home in the summer mailing.  
• Health Appraisal Form - required for all students new to the school, students moving to a new learning community, and for middle and Upper School students participating in the athletic program; completed by parent/guardian and physician.
  • ECP-Grade 8 Health Appraisal Form 
  • Grades 9-12 Health Appraisal Form  (This is the standard MHSAA form.)
• Asthma Emergency Action Plan - required for any student with asthma; completed by parent/guardian and physician
• Authorization for Long-Term/Short-Term Prescription Medication - required for any student taking prescription medication during the school day; to be completed by parent/guardian and returned with labeled medication
• Authorization for Over-the-Counter Medications - ECP-Grade 1, Grades 2-4, Grades 5-12 - required
• Food Allergy Action Plan - required for any student with severe allergies; completed by parent/guardian and physician  
• ImPACT Consent Form - Required for new students in the middle and Upper Schools, including all students in Grades 5 and 9.
